1-1.Birth Registration
When a baby is born in Japan, submit a “Birth Notification” to the municipal office.
The “Birth Notification” must be submitted within 14 days after childbirth.

1-2.Residence Card
When a baby born in Japan
(1)has a nationality other than Japan, and
(2)will stay in Japan more than 60 days after birth
Obtain the “Residence Card” for the newborn baby.
Within 30 days after birth, bring the documents from the city office to Sendai Regional Immigration Services Bureau (which is in charge of the procedure for residency examination).

1-3.Report to your home country
Report the birth of the baby to your home country
You have to report the birth of the baby to your home country even when your baby is born in Japan.
Deadlines for procedures and required documents differ from country to country.
Contact the embassy of your home country in Japan.
2.Newborn’s Birth Report
Submit the Newborn’s Birth Report together with the Birth Registration to the municipal office.
The Newborn’s Birth Report Card is enclosed in the “Health Guide for Mother and Child” that you have received with Maternal and Child Health Handbook.
The card is used for “Hello Baby Visit” by a public health nurse or midwife in Motomiya City.
During the “Hello Baby Visit”, your baby’s weight is measured and you can receive childcare consultation.

3.Procedures for health insurance
A baby will enroll in the health insurance in which their parents are enrolled in.
See Page to confirm in which insurance you enroll.
In case of National Health Insurance
The procedure is done in the municipal office.
In case of Social Insurance
The procedure is done in the parent’s workplace.

4.Application for Child Allowance
Those who raise children in Japan receive “Child Allowance”
You need to apply for it at the municipal office within 15 days after birth of your child or after you move in to different municipality.
The amount in the table below per month is given in three installments over a year. The amount of allowance varies by income of a parent.

5.Medical expenses for children
Go through the procedure to receive “medical expenses for children”
Motomiya City subsidizes hospital bills from the time a child is born until the child turns 18 years old.
The procedure varies by the health insurance that the parents enroll in.

6.Baby health checkup
The municipal office provides baby health checkup to check the size and physical condition of infants.
It is free of charge.(Free of charge)
You can receive consultations about the baby’s growth and what you want to know.
Your baby goes through the checkup on a specified schedule. Come for the checkup after you receive the notice.

7.Vaccination
Vaccination (injection to prevent illness) is available.
Your child will receive vaccinations that are required by national law.
It is free of charge. (Free of charge)
The type and the number of injections of vaccination are specified according to the age of a child.
Make sure to receive vaccinations when your child is able to have injection.
