Necessary procedures when moving out of Motomiya City
When you determine the date to return to your home country or to move out of the city, you need to inform the landlord, electricity, gas, water supply and telephone company and banks.
Then, go through the respective cancellation procedure.
On the week of your returning to your home country or moving out of the city, complete the procedures to move out of Motomiya City at the municipal office.
When moving out of Motomiya City to live other municipality, complete the move-out procedure in the Motomiya City and then go through the move-in procedure in the new municipality.

Only those who enroll in the National Health Insurance are required to go through the procedure in (3). See Page to confirm which insurance you enroll in.
Only those who enroll in the late-stage elderly medical care insurance are required to go through the procedure in (4).
(5)Municipal tax Even if you return to your home country in the middle of a year, you must pay the municipal tax for that year.

Pay the tax at the municipal office or a bank. - Those who return to their country before the tax notice is sent
Have someone in Japan to be your “tax manager” to pay on your behalf.
Before you return to your home country, you must go through the procedure to appoint your “tax manager” in the municipal office.
* Lump-sum Withdrawal Payments for the National Pension Plan
Those who have paid for insurance in Japan for more than 6 months are entitled to receive a lump-sum withdrawal payment if they complete the procedure within 2 years of their return to Japan.
Before you leave Japan, confirm with a nearby pension office
To have someone in Japan to do the procedure on your behalf, you will need to prepare a power of attorney (a document that requests to allow the person to do the procedure on your behalf).
